エストロゲン薬は以下のような目的で使用されています ② 陰門、膣の萎縮症の治療 ③ 性腺刺激ホルモンの低下による女性ホルモン不足の治療、去勢、初期卵巣不全の治療 ④ 適切に選ばれた転移した病気をもった男性・女性の乳がんの治療（一時的緩和用） ⑤ 進行性男性ホルモン依存前立腺ガンの治療（一時的緩和用） ⑥ 骨粗しょう症の予防 骨粗しょう症とは、骨が細くなるため、弱く壊れやすくなってしまう症状を言います。 脊椎、手首、臀部の骨が最も症状の出やすい場所です。 エストロゲン置換療法は、骨の再吸収を減らし、更年期を迎えた後の骨の損失を妨害または停止させます。 ケースコントロール研究によると、更年期にはいり2、3年のうちに、エストロゲン置換療法を行った女性では、臀部や手首の破砕は約60％も減少したことが分かりました。 また、エストロゲンは脊椎の破砕も減らすことが分かりました。 さらに、更年期に入り6年も経ってからエストロゲン置換療法を開始しても、治療を続けている限り、骨の破砕を防げることが分かりました。 治療を停止すると、更年期を過ぎた頃の状態まで、骨は劣化してしまいます。 エストロゲン治療によって、更年期の前の状態まで骨の状態を戻すことはまだ証明されていません。 有色人種に比べ、白人やアジアの人は骨粗しょう症にかかる危険性が一般的に高いといわれています。 早期更年期症状は、骨粗しょう症の進行のもっとも明確な予兆のひとつです。 さらに 遺伝的要素（小柄、家系）、内分泌要素（未産、甲状腺中毒症、上皮小体過多症、カッシン氏症候群、催乳ホルモン過多症、タイプＩ型糖尿病）、生活習慣（喫煙、アルコール中毒、運動不足）、栄養（低体重、食事からのカルシウム摂取量）などの、他の要素も骨粗しょう症に関連する骨格に影響を与えます。 骨粗しょう症の予防・管理の柱となるのは、エストロゲン、適切なカルシウム摂取、運動です。 更年期を過ぎた女性は、更年期前の女性に比べ、カルシウムの吸収率が落ちますので、栄養のバランスを保つために1日に1500ｍｇのカルシウム摂取が必要とされています。 散歩やジョギング等の運動や、正しい栄養摂取は骨粗しょう症の予防にたいへん重要なことです。 ガン発生の危険因子ともなりますから、極少量の服用を強く薦めます。 いかなる治療を受ける場合でも必ず医者に相談し、この薬を服用している間は、定期的に胸の検査を受けるようにして下さい。 さらに詳しくお知りになりたい方は、「プレマリンに関するよくある質問とその答え」を参照してください。 服用法：食事と一緒でも別のときでも構いません。 毎日1錠（0.625ｍｇ）ずづ服用します。 できるだけ、毎日同じ時間に服用するよう心がけてください。 まず25日間続けて服用します。 その後5日間は服用を停止します。 このサイクルを繰り返します。 医師の診断を受けずに長期にわたり服用を続けてはいけません。 製造元：ワイエスーアイエルスト 内容量：0.625ｍｇ 100錠 メーカー純正密封容器入り。 プレマリンは、天然資源より得られたエストロゲンの混合物です。
Premarin (Conjugated Estrogens) : Osteoporosis
Estrogen drug products are indicated in the: 2. Treatment of vulvar and vaginal atrophy. 3. Treatment of hypoestrogenism due to hypogonadism, castration or primary ovarian failure. 4. Treatment of breast cancer (for palliation only) in appropriately selected women and men with metastatic disease. 5. Treatment of advanced androgen-dependent carcinoma of the prostate (for palliation only). 6. Prevention of osteoporosis. Osteoporosis is a thinning of the bones that causes them to weaken and break. The bones of the spine, wrist, and hips seem to be the most affected. Estrogen replacement therapy reduces bone resorption and retards or halts postmenopausal bone loss. Case-control studies have shown an approximately 60 percent reduction in hip and wrist fractures in women whose estrogen replacement was begun within a few years of menopause. Studies also suggest that estrogen reduces the rate of vertebral fractures. Even when started as late as 6 years after menopause, estrogen prevents further loss of bone mass for as long as the treatment is continued. When estrogen therapy is discontinued, bone mass declines at a rate comparable to the immediate postmenopausal period. There is no evidence that estrogen replacement therapy restores bone mass to premenopausal levels. White and Asian women are at higher risk than black women. Early menopause is one of the strongest predictors for the development of osteoporosis. In addition, other factors affecting the skeleton which are associated with osteoporosis include genetic factors (small build, family history), endocrine factors (nulliparity, thyrotoxicosis, hyperparathyroidism, Cushing's syndrome, hyperprolactinemia, Type I diabetes), lifestyle (cigarette smoking, alcohol abuse, sedentary exercise habits), and nutrition (below average body weight, dietary calcium intake). The mainstays of prevention and management of osteoporosis are estrogen, an adequate lifetime calcium intake, and exercise. Postmenopausal women absorb dietary calcium less efficiently than premenopausal women and require an average of 1500 mg/day of elemental calcium to remain in neutral calcium balance. Weight-bearing exercises like walking or running and nutrition are important to the prevention of osteoporosis. Since there is a risk factor in the development of certain cancers, maintaining a low milligram intake is strongly suggested.
